GLP-1 Weight Loss and Body Contouring

The GLP-1 Effect on Weight Loss

GLP-1 medications and semaglutides have quickly become one of the most talked-about developments in weight loss. Dr. Chad Robbins sees the impact firsthand and calls them a turning point in how the country approaches obesity and excess weight. This is only the first generation of these drugs, and future versions are expected to be even more accessible, including oral options.

With the rise of these medications has also come a rise in loose skin and overall volume loss. In this video, Dr. Robbins explains what happens to the body after major weight loss and how body contouring can help patients see results that match the progress they’ve already made.

What Happens to the Body After Significant Weight Loss

Whether weight loss comes from a GLP-1 medication, bariatric surgery, or diet and exercise alone, the aftermath on the body tends to look the same: excess skin and lost volume, particularly in the face, breasts, and buttocks. These are the areas most people want to keep full, since fullness there is closely tied to a youthful, balanced appearance.

Dr. Robbins has spent years treating patients through this exact stage of the process, and body contouring after weight loss has become one of the primary focuses of his practice.

The Two Most Common Areas: Breasts and the Trunk

The two procedures Dr. Robbins performs most often after massive weight loss involve the breasts and what he calls the trunk, the circumferential skin excess around the torso and flanks. The term “trunk” is used deliberately instead of “abdomen” because abdominoplasty alone doesn’t address the excess skin that persists along the sides and back after major weight loss.

Breasts are made up of skin, breast tissue, and fat. With weight loss, patients often experience glandular atrophy and loss of fat, leaving breasts deflated with nipples positioned lower than desired.

One important distinction: traditional breast ptosis, or drooping, is measured by the position of the nipple relative to the crease under the breast. After massive weight loss, that crease itself often slides down the torso, changing the whole shape of the breast. This has shifted the techniques Dr. Robbins uses, including procedures that raise and reset the inframammary crease higher on the chest, along with addressing excess skin on the lateral chest wall. Without correcting the position of the crease, even a well-placed implant won’t create the fuller, higher look most patients want.

Why These Consultations Take Time

Patients who come in after significant weight loss often have a long list of concerns. Consultations frequently cover the entire body, from facial aging and interest in facelifts at younger ages, to skin excess on the arms, breasts, back, torso, and thighs.

Plastic surgery has strong solutions for many of these areas. Others remain more difficult to treat. Saddlebags and the crepey skin that gathers just above the knee, for example, still don’t have great surgical solutions.

Building a Personalized Treatment Plan

Because patients often present with numerous concerns at once, Dr. Robbins and his team focus on creating the most meaningful improvement with the least amount of risk, recovery time, and cost. Not every concern can be addressed safely in a single treatment plan, so priorities are set based on each patient’s anatomy and goals.

Beyond the breast and trunk, common procedures for post-weight-loss patients include:

  • Facelifts, neck lifts, brow lifts, and eyelid surgery
  • Arm lifts and thigh lifts
  • Breast augmentation and volumizing procedures
  • Gluteal auto augmentation, using the patient’s own excess tissue to add volume to the buttocks
  • Liposuction, though this addresses fat rather than skin excess

Gluteal auto augmentation has become more common in patients who don’t have enough fat remaining to serve as a donor site for a traditional Brazilian butt lift, offering another option for patients seeking a fuller, more contoured result using their own tissue.
